Download the change of name form from here and submit it in person at the SNMC secretariat with :

  • Completed Change of name application form
  • Original marriage certificate or divorce decree (A certified copy will be made)
  • National Identification Card

Submit the following by  post or in person:

  • A Letter to the Registrar or A signed letter
  • Prescribed fee Applicable
  • Submit to:

Registrar, SNMC Office, Blue roof building,  Ministry of health, Mont Fleuri, Mahe, Seychelles

Indicate this on the  license Renewal Application Form by selecting the option “Do not wish to renew. Submit the form to the SNMC.

Nurses must update their contact details every three years to remain on the register.

A list of registered  Nurses and midwives is published  annually in the Seychelles Gazette www.gazette.sc
